A match point for better care
Lifen’s connectors work behind the scenes to ensure all Moovcare accounts match existing facility patients. To do this, Lifen Platform relies on an HL7 data standard “Admit-Discharge-Transfer” flow that connects Moovcare to the facility’s Patient Administration System. The application can thus retrieve all identifying patient information and link the two accounts. The care team don’t need to fill in any fields manually, and neither does the patient.
Questions that save lives
Patients regularly receive questionnaires on their Moovcare app, which they can fill in only minutes. Their answers are compiled into a PDF report and automatically ingested into the facility’s EHR queue, this time using an HL7 “Medical Document Management” type of flow.
A bird’s eye view
The primary care team now has optimal visibility of the latest developments in patients under their care. By reviewing the reports, they can spot potential complications earlier, and intervene as necessary. Once they have acknowledged the report, they can feed it into the system of record in just one click.
